POSITION SUMMARY

The Behavioral Health Consultant will be a core member of the interdisciplinary Patient Centered Primary Care Team. The licensed clinician will possess excellent clinical skills and provide behavioral health care using evidence-based treatment models with adults, children, and families. The BHT will establish and implement clinical treatment goals in collaboration with the integrated health care team.

CORE JOB RESPONSIBILITIES
Essential Functions
·         Works collaboratively as part of an integrated health care team with providers including, but not limited to, Physicians, Nurse                              Practitioners, Nurses, Mental Health Counselors, Clinical Psychologist(s) and Psychiatrist(s).
·         Provides high quality, evidence-based assessment, diagnosis, brief intervention and referral to treatment behavioral health services as            appropriate based on client’s unique clinical presentation. Treatment modalities include, but are not limited to, individual, family, and               group therapies at any of the CCHC sites.
·         Provides crisis intervention and conducts warm hand-off assessments as needed.
·         Accurately documents all clinical interventions, diagnosis, symptoms, and presenting problems in the Electronic Medical Record in                  accordance with federal, state, and local regulations.
·         Works with patient to develop treatment plan/goals, based on social, emotional, and health needs of the patient/family.
·         Offer long term therapy as appropriate with related documentation.
·         Incorporates health and wellness education and referrals to preventative care.
·         Coordinates care with patient’s care team and communicates regularly with care team.
·         Participates in peer and chart review.
·         Participates in individual and group clinical/administrative supervision.
·         Complies with established departmental policies, procedures, and objectives.
·         Attends a variety of meetings, conferences, and seminars as required or directed.
·         Demonstrates use of Quality Improvement in daily operations.
·         Complies with all health and safety regulations and requirements.
·         Maintains patient confidentiality and complies with HIPAA regulations.
·         Respects diverse views and approaches, and contributes in maintaining an environment of professionalism, tolerance, civility, and                    acceptance toward all employees, patients, and visitors.
·         Performs other similar and related duties as required or directed.
·         Demonstrates a patient-center and trauma-informed approach to care.

 QUALIFICATIONS:
Work Experience
·         Minimum of two (2) years of experience providing clinical treatment to families and children, preferably in the health care field, in                      individual, family, group, and crisis intervention modalities.
·         Experience in the provision of assessment, diagnosis, service planning, care coordination, referral, and brief intervention services for                various mental health conditions, including substance abuse or dependency and dual-diagnosis presentations. 
·         Experience using electronic medical records, Microsoft Office products, and computers
    
Education, Training, Certification and Licensure
·         Required: Master’s degree in social work, counseling, or related field
·         Preferred: Current State of Oregon license as a Licensed Clinical Social Worker (LCSW).
 
Skills, Knowledge & Abilities:
·         Must be able to interact with a variety of individuals at various levels under stressful circumstances while exercising sound judgement,            tact, and diplomacy.
·         Knowledge of evidence-based approaches to brief intervention
·         Knowledge of basic medical terminology preferred.
·         Knowledge and experience using the current DSM and/or ICD codes.
·         Demonstrated ability to work as part of an integrated health care team, and to establish and maintain rapport with patients, clinic staff,           and co-workers from diverse backgrounds.
·         Knowledge of HIPAA and 42-CFR regulations.
·         Ability to work with people in various environments.
·         Must be able to work independently, communicate medical information, and perform health interventions in a clinical setting.
·         Communicate with clarity, both verbally and in writing, to groups and individuals.
