As part of the Behavioral Health Team, the positions bridges the gap in care for patients of our primary care team who may either be working with our behavioral health team, or have complex prescription needs associated with psychiatric or psychotropic medications. Pivotal to supporting this team, this position will report to the Director of Behavioral Health, and liaise with the primary care and population health teams to consult on patient care which may include assessment, diagnosis, and management of mental health problems and psychiatric disorders. 
 
Essential Functions:
•                  Performs a psychiatric assessment (including evaluation of mental status, current and past history of violence, suicidal or self-harm behavior, substance use, level of functioning, health behaviors, trauma, sexual behaviors)
•                  Diagnoses psychiatric disorders
•                  Differentiates psychiatric presentations of medical conditions from psychiatric disorders and arranges appropriate evaluation and follow-up
•                  Evaluates potential abuse, neglect, and risk of danger to self and others and assists patients and families in securing the least restrictive environment to ensure safety
•                  Conducts individual, group, and/or family psychotherapy
•                  Treats acute and chronic psychiatric disorders and mental health problems
•                  Plans care to minimize the development of complications and promote function and quality of life using various treatment modalities such as psychotherapy and psychopharmacology
•                  Prescribes psychotropic and related medications based on clinical indicators of patient status to treat symptoms and improve functional health
•                  Manages psychiatric emergencies by determining level of risk, and initiating and coordinating effective emergency care
•                  Participates in community and population-focused programs that promote mental health and prevent or reduce risk of psychiatric disorders (for example: self-esteem, healthy relationships, coping without substances, stress management, memory deficits, grieving, etc.)
•                  Provides psychoeducation to individuals, families and groups to promote knowledge, understanding, and effective management of mental health problems and psychiatric disorders
•                  Makes appropriate referrals and coordinates transitions to other health care professionals, agencies and community resources for individuals and families
•                  Advocates for access and parity for mental health problems, psychiatric disorders, and addiction services
•                  Collaborates as a member of the interdisciplinary team
•                  Provides consultation to health care providers 
•                  Uses the therapeutic relationship to promote positive clinical outcomes
•                  Identifies and maintains professional boundaries to preserve the integrity of the therapeutic process
•                  Seeks consultation actively
•                  Refers patients requiring more specialized or complex treatment to the most appropriate resource

Other Duties and Responsibilities of the Psychiatric/Mental Health Nurse Practitioner:
 
•                  Participates in staff conferences and serves on committees under the direction of the Medical Director
•                  Completes medical records and reports in a timely manner
•                  Coordinates with support staff to ensure continuity of care 
•                  Performs other duties as assigned
  
Education and Experience:
 
•                  Bachelor’s and Master’s degree in Nursing from an accredited college of nursing
•                  Completion of an accredited P/MHNP program
•                  Oregon P/MHNP license with no adverse action
•                  Oregon prescriptive authority
•                  A minimum of five years experience in a primary care setting
•                  Experience in a rural setting is preferable
•                  Maintain all active professional license and complete continuing education as required.

Physical Demands:
While performing the duties of this job, the employee is frequently required to sit and talk, listen, handle objects, tools, controls, and equipment, Employee must reach and bend, stand and walk, kneel and bend extended periods of time. The employee must occasionally physically assist persons, lift and/or move more than 25 pounds.
